Better Late Than Never: ISBE FAQ Document

ISBE has created a Frequently Asked Questions (FAQ) document addressing the recent changes to Forms 34-54 (IEP forms) and 34-57 (Notice and Consent forms) as required by the 2024 review. These newly revised forms were posted on ISBE's Special Education webpage back in September and October, 2025. In the linked review, you will see references to laws and regulations in the column on the right. The following abbreviations are used: CFR=Code of Federal Regulations; ILCS=IL Compiled Statutes; IAC=IL Administrative Code. The CFRs are the federal regulations we must uphold, and the ILCS and IAC are the state laws we must uphold. ISBE Responses to Questions Regarding 34-54 and 34-57 can also be accessed through ISBE’s Special Education Department webpage.
Your district’s online IEP forms from platforms such as Embrace, Frontline Education (i.e.EasyIEP), or PowerSchool, may look slightly different, but all documents should have the same content as ISBE’s 34-54 or 34-57 forms.
